A clinical study on pyogenic liver abscess


Abstract
This clinical study is based on a review of the records of 73 patients with liver abscess including pyogenic and amebic abscess that were confirmed by percutaneous aspiration or surgical procedures at Pusan National University Hospital during the
period
of 5 years from January 1988 to December 1992.
@ES The results were summarized as followings :
@EN 1) Of the 73 cases of liver abscess, 59(80.8%) were pyogenic, 11(15.1%) were amebic, and 3(4.1%) were mixed.
2) Peak age incidence was fifth decade in case of liver abscess, and forth decade in case of amebic abscess.
3) In sex incidence the male to female ratio was 1.4 : 1 in pyogenic abscess and 2.7 : 1 in amebic abscess.
4) The right lobe of liver was more commonly involved than left lobe in 66.1% in pyogenic abscess and 72.7% in amebic abscess.
5) Multiple abscesses were noted in 49.3% in liver abscess, and 54.2% in pyogenic abscess and 27.3% in amebic abscess.
6) The most common clinical symptoms were chill(67.1%) in liver abscess, and fever(96.6%), chill(74.6%) in pyogenic abscess and fever(81.8%), right upper quadrant pain(63.6%) in amebic abscess.
7) Leukocytosis in the liver abscess occurred in 78.0% in pyogenic abscess and 72.8% in amebic abscess and the most frequent abnormal liver function test is increase in serum alkaline phosphatase in 44.1% in pyogenic abscess and 36.4% in amebic
abscess.
8) Operative drainage was performed in 64.4% and percutaneous aspiration in 35.6% among 73 cases of liver abscess.
9) On bacteriological culture, rate of positive culture was in 47.5%, E. coli was the most common organism.
10) Complication occurred in 25 cases(34.2%) of case of liver abscess after drainage procedures and mortality rate was 8.2% of 73 cases.
